West End Support Services is committed to protecting your personal information. This policy explains what we collect, why we collect it, and how we keep it safe, in line with the Privacy Act 1988 (Cth), the Australian Privacy Principles and the NDIS Code of Conduct.
We only collect the information we need to support you safely and deliver our services: your name and contact details, NDIS plan and participant details, health and support needs, emergency contacts, and records of the supports we provide. For staff and job applicants, we collect the information needed for employment, screening and training. If you use our website or enquiry forms, we collect the details you choose to send us.
Your information is used to plan and deliver your supports, keep you safe, meet our obligations to the NDIA and the NDIS Quality and Safeguards Commission, process claims and payments, and improve our services. We never sell your information, and we never use it for marketing without your consent.
We only share your information with your consent, or where the law requires it: with the NDIA and the NDIS Commission, with health professionals involved in your care, with your nominated family members, guardians or advocates, and with government bodies where mandatory reporting applies. Anyone we share with is bound by the same confidentiality standards we hold ourselves to.
Records are stored securely, with access limited to the staff who need them to support you. Our team is trained in privacy and confidentiality, and records are retained and disposed of in line with legal requirements.
You can ask to see the information we hold about you, ask us to correct it, and ask questions about how it is handled at any time. You can also withdraw consent, and we will explain any effect this may have on your supports.
